What is the CMS ACCESS Model?

ACCESS (Advancing Chronic Care with Effective, Scalable Solutions) is a voluntary CMS Innovation Center model that tests an outcome-aligned payment approach in Traditional Medicare to expand access to technology-supported chronic care. The model runs for 10 years, beginning July 5, 2026.

Core Participation Requirements

  • Active Taxpayer Identification Number (TIN)
  • Compliance with applicable state licensure, HIPAA, and FDA requirements
  • A designated physician Clinical Director responsible for clinical and compliance oversight
  • Enrollment in Medicare Part B as a provider or supplier
  • Ongoing outcome reporting — CMS will maintain a public directory of participants, the conditions they treat, and risk-adjusted outcomes
